# sk65629 - Errors when adding IPS exceptions in SmartDashboard - "Failed to revert object", or "Failed to save object"

## Symptoms

- * When adding an IPS exception in SmartDashboard, an error message appears:

  * `Failed to revert object.Server error is: N/A. (Code: 0x800415A1, Object Not Found)`  

  * `Failed to save object.Server error is: Update without rename operation. (Code 0x8003001D, Could not access file for write operation)`
* Debug of FWM daemon (per [sk86186](http://supportcontent.checkpoint.com/solutions?id= sk86186)) shows:

  `table ips_exceptions has no column named profile_name`

## Cause

A corruption has occurred in the IPS database files.
