# sk180921 - "Failed to get modules list" error when adding a Domain Management Server to Algosec

## Symptoms

- * When you add a new Domain Management Server to Algosec, Algosec logs this error:   
  "`Failed to get modules list`"

* This error appears in the `cpm.elg` file on the Multi-Domian Server after you try to configure the Domain Management Server on the Algosec server:   
  "`More than one result was returned from Query.getSingleResult(); nested exception is javax.persistence.NonUniqueResultException: More than one result was returned from Query.getSingleResult()`"

* Notes:
  * The connection with the Algosec server uses OPSEC.
  * You defined an OPSEC Application object with the correct services.
  * You created an administrator user on the Multi-Domain Sever and configured its correct credentials on the Algosec server. The Multi-Domain Sever audit logs show successful logins/logouts for the admin account the Algosec server uses to query the Domain Management Serve for data.
  * The SIC status of the OPSEC server is "Trust established".

## Cause

The administrator user account and the OPSEC Application have the same name, which prevents the Multi-Domain Sever from responding correctly to the Algosec server's queries.
